New patient information
What to expect at your first visit, what to bring, forms, and our appointment and cancellation policies.
Your first visit
Plan on 90 minutes. You will have a full set of X-rays, a comprehensive exam with the dentist, a gum health check, an oral cancer screening, and a cleaning if time allows and your gums are healthy enough for a regular cleaning. You leave with a written treatment plan and estimate, even if the plan is just 'see you in six months.'
What to bring
- Photo ID
- Insurance card, or the insurance company name, group number and subscriber ID
- List of current medications
- Previous X-rays if you have them (or tell us your previous dentist and we will request them)
Forms
New patient forms are emailed and texted to you when you book. Completing them online before your visit saves about 15 minutes. Paper forms are available if you arrive 15 minutes early.
Appointment policies
- We confirm by text and email 48 hours before every appointment. Reply C to confirm.
- Please give 24 hours notice to cancel or reschedule. A second missed appointment without notice carries a $50 fee; a third moves you to same-day-only scheduling.
- If you arrive more than 15 minutes late we may need to shorten or reschedule the visit.
- Children under 18 must be accompanied by a parent or guardian for the first visit and any visit involving treatment.
Transferring from another dentist
Sign a records release at your first visit or ask us to email you one. Most offices send X-rays within a week. There is no charge to transfer.
